2.4 Winner Selection

Standard dental consent documentation must be signed prior to treatment.

2.7 Publicity

Both photos must:

Clearly show upper and lower teeth

Failure to provide sufficient evidence will result in claim rejection.

3.3 Usage Requirements

To qualify:

Unused product does not need to be returned.
